I think this is the key point in this whole discussion. Yes, the
offending faculty member should be required to deal with his or her
department regarding the act of forgery, but the IRB needs also to take
action that will insure that the human subjects review requirements and
procedures are taken seriously by all those involved in research. The
review of all proposed research in a timely manner and with proper
sign-off is crucial to insuring integrity in research at the institution.
