Airfare upgrades for medical reasons

I have a PI on an NIH grant who says he has a medical condition that makes flying in economy class very painful. I seem to remember that there is a stipulation that allows PIs to upgrade their tickets if it's medically necessary, but I can't find the back up. Any chance someone knows where I can find the reference to this exception, the medical conditions to which it refers, and the backup we need to have on file?
