Pat,

Before you can design a survey, you have to know exactly what questions
you want answers to. We're talking about writing measurable objectives.
It's pretty unlikely that someone else's survey will meet your needs.
Unless you or your staff have training in survey design and evaluation,
I suggest you turn this over to a unit on your campus that does. You're
also going to run into huge bias problems if you design and evaluate
your own survey. It's worth the price you'll pay to have this done
right.
