UAH has a conflict of interest/financial disclosure form which must be
signed for each proposal relative to the PI and the proposed funding agency.
If we receive an award without a proposal processed through this office (I
know it's hard to believe this ever happens), the form must be completed
before we will process the award.

A similar form must be signed by a PI who is requesting us to issue a
subcontract under his/her award relative to the PI and the consultant or
subcontractor.
