Nice try, but this saves the form without the fields filled in, which is I believe what the original question was.

The only way to do this, as has been stated, is to use Acrobat.  An alternative is to use the Word version of the form.  (Some agencies - NIH - are using an RTF version, somewhat more universal than Word.)
