While your description of the situation is much more clear than the NIH notice, many issues remain.  Aside from the fact that each institution is best able to make many simple and not so simple adjustments to the forms to make it easier for THEIR applicants, the larger issue concerns timing of NIH changes to their own forms.
A week before a deadline?  A month?  3 months?  There are many people out there working on applications for the 1 October deadline who have already started using the 5/01 forms.  Urging applicants "to always check the PHS 398 and PHS 2590 websites" presumes a lot.
