Consider it the same as a consultant letter and place just before the Checklist.

However, Letters of Reference per se are not usually requested (e.g. for an R01), and in the case of a renewal, I'm not sure that testimonials mean very much.  In fact, a reviewer might see it as a way to divert his/her attention from poor progress or publication during the previous project period.

You might consider making the writer an unpaid co-investigator or consultant who in the course of agreeing to offer guidance or help during the project says what a terrific project it is and how it fits in well with the already stellar accomplishments of the (young?) applicant.
