Had the same problem and the piece meal offered up by the usual GPO access
is virutally useless. I contacted GPO and received the following
(non-intuitive, torturous) scheme for retrieving a citation as a complete
document.

Using the GPO Acess site in Washington, DC:
http://www.access.gpo.gov/nara/cfr/index.html

1.      Select "Search or browse your choice of CFR titles and/or volumes
(current and/or historical data)"
2.      Scroll down to Title 45, select box for October 1, 1999
3.      Scroll down and hit the "Continue" button
4.      Select 1, enter "74" as search terms
5.      Deselect Revised October 1, 1999 box

Among the search results will the 45CFR74. Right-clicking on the PDF icon
allows one to download and save the file locally (much preferred to opeining
online). One can refine the search even more to select sections such as
Appendix E.

Not straightforward, but it works...Bill

Can someone share with me a good web site address for the cost principles
for hospitals, 45 CFR 74, Appendix E (and NOT the on-line piece-by-piece
database for the Code of Federal Regulations -- this does not work well for
me).

I had a good site on DHHS bookmarked, but of course their web site has been
revamped and I can't find it anymore.
