I am trying to locate a copy of the DD Form 441 entitled Certificate
Pertaining to Foreign Interests.  I have thus far been unsuccessful
locating this document and unfortunately have a short lead-time to submit
with a proposal.  Any assistance locating this form would be greatly
appreciated.
