I also had to get this form today for a NASA proposal. The form I used can be found on the ED website:

Look for the ED 80-0013 Form

Certifications Regarding Lobbying; Debarment, Suspension and Other Responsibility Matters; and Drug-Free Workplace Requirements, ED 80-0013 form (12/98)

HOWEVER, this form - ED Form GCS-008 (Rev. 12/88) - is an old version.

We are applying for our first grant through NASA.  Actually, I think it is a
consortium agreement and the only item that NASA is requiring from us at
this time is the Debarment Certification and Drug-Free Workplace
Certification.

Most of our grants are with NSF and NIH; therefore, the certification form
has been part of the NSF or NIH application package. Hence, I've never had
to prepare such a certification page for any other federal agency.

Is their a site on-line where I can find such a certification page?  Or
could you provide me with an example of such a certification page that NASA
will approve? Thanks in advance for any assistance you can offer.
