Time and Effort Reporting

I am interested in learning how different Institutions deal with Time
and Effort Reporting. In particular, I would like to hear from Hospitals
in regards to the following issues:
1. Do you require certification of Effort for everyone paid from a
Sponsored Project or only key personnel? If you only require
certification for key personnel, has this created problems? If
certification is required for everyone, has this created problems?
2. Is certification required monthly, quarterly, or some other time
period? Whatever time period you use for certification, does it work
well, or do you believe a different time period would work better? Have
you always used the same time period or have you adjusted the time
period?
