Thanks for the replies.  It's true, it was a "customer satisfaction" survey.
(Actually, concerning treatment programs, but details ruin the story).
FYI The surveys were not anonymous, facility did not obtain informed
consent, student is not doing research at this facility.  IRB, however, is
ready for prison projects.  RESADM-L comes through again.  Thanks.
