At my previous job, we scanned forms into OmniForm and then converted them
to PDFs. The forms could be filled out online, but had to be printed out to
be used--so I'm not sure they qualify as "internet-ready." We put both
file types on our web site, but found PDFs were easier for people to manage
and understand. But OmniForm was great in that it automatically created
the entry fields. Their website is
