One of the things I’ve encountered is a hidden space within the template – specifically in the middle name field. You might try to see if that is the problem.

I suspect it could be something in the underlining coding.  If the information you are pasting is bold or in italics, then that code could be the problem.  I’ve run into this when trying to cut and paste protocol information in Human Subjects Study Record fields.  I’ve cleaned that up in 2 ways:

  1. Copy your information into a blank Word document.
    1. Highlight the text.
    2. Under the Font area, find this symbol  and click on it.
    3. That should remove all the formatting.
  2. Copy your information into a blank Word document.
    1. In the paragraph area, find this symbol  and click it.
    2. The document will now show underlying coding such as paragraphs, tabs, page breaks, returns, etc.
    3. Delete any coding related to formatting.

                                          i.    This is a screen grab from my submission confirmation document so you can see what I mean.

                                         ii.

    1. Don’t forget to unclick that paragraph mark in the menu or all the documents you open in Word after that, will show this kind of information.

Hope this helps.
