We do similar, only with Microsoft Forms, which feeds into our Teams channel.   I have created a publicly accessible version if you’d like to preview what ours looks like. 

 

We then dump (via simple cut and paste) that data in our proposal checklist, which our Proposal Coordinators use to guide the proposal development   #WorkSmarterNotHarder

We also use Google Forms for proposal intake, like a survey. 

 

It asks questions about the PI, department, submission details, collaborators and compliance questions such as human subjects, animals, foreign activity.

For proposal intake, I set up a survey. My university uses Qualtrics and I created a survey for the PI to fill out. I do expect to have to re-create it in a few years when the university changes its survey platform which it seems to do every few years. Before that we used a fillable pdf form but that was always plagued with annoying quirks.

 

The key to making it handy is to set up the questions in the intake form to mirror whatever system you are entering the information into. So if the system asks for the RFP link first, the project dates second, and the name of the PI third, then the intake form asks the questions in that same order.

As much as I don’t want to be a “me too” email to clog up the list serve, I am also interested in any templates other institutions would be willing to share.  We are in the process of revamping our procedures because of the exponential growth in research activity we have experienced. 

 

Procedures/policies I would be interested in are:

                Proposal intake forms (when a PI notifies the OSP of an intent to submit)

                Internal review of proposals

                Award/contract checklists
