I am sharing a PHS Human Subjects and Clinical Trials Information guide. The first page is a decision tree with hyperlinks to the table of requirements based on the human subjects determination from the bottom of page 1. The tables then
list the required documents based on project parameters. Finally, the annotated forms are at the end of the document as reference.
