We recently submitted an HRSA CDS Application.  Our application only included a few documents.  We received guidance during the HRSA webinar and the frequently asked questions on their website.  Our application included the following documents:

  1. Budget
  2. Budget Justification
  3. Budget Narrative
  4. Project Narrative
  5. Project Abstract 
Since our project did not include true construction costs, we did not need to include drawings, etc.
