We used to have a proposal intake form and we recently incorporated it into our GMS and got rid of it as a separate form. One of the best things about this change
is the reduced amount of double data entry. If you are able to go this route, I highly recommend it.

We use one in our department. It’s a Google form, and we have it configured so the PI has to select their fund manager and the system then sends an email to the person they select with all of the answers. The form also then populates
a Google sheet with their entry, which creates a useful log of all proposal submissions. The only drawback is having to repeatedly remind faculty to use the form, but I’d say we’ve gotten to about 80% adoption without reminders (i.e. they just go there to
notify us of an upcoming proposal without emailing their fund manager first).
I’m attaching a PDF printout of the questions on ours. If they answer yes to the subawards question at the end, it gives them another question to provide the subrecipient institutions, PIs, and admin contact info.

We are considering using a proposal intake form at our university to help get a better understanding of the complexity of each grant at the beginning of the process. This form would includes items
such as is there cost share, sub recipients, etc. I was wondering if any of you use/have used proposal intake forms and if so did you find them helpful? Were there any drawbacks to using them?
