I am on a committee tasked with overhauling our entire IRB process. We are 100% tearing it down and rebuilding. Would anyone else from a small liberal arts college (meaning little/no medical research concerns and high number of social science proposals) be willing to share their IRB forms so we can get an idea of best practices? We would never just use anyone’s with modification/prior permission.

 

Also, does anyone have a simple (meaning Excel or Access) database for tracking protocols?
