this sounds like a good chance for "crowd sourcing."  If all who review the document would post their comments and changes in one of the "sharing" apps (Google Docs, Windows live, or something) then a great template could be developed.  You could use something like the "commons" license to let all contribute and receive appropriate credit for their effort. 
Maybe a real opportunity for a student or retiree with a bit of time on their hands to set it up.
