I need guidance on preparing a contract for a faculty member who will be providing services (acupuncture) for an integrative medicine program at a local hospital but may also collect data from the patients as part of her research program (with IRB approval).  Have any of you seen such an agreement and if so would you be willing to provide me with a template?

This listserv is such a great resource.  I learn so much without even asking any questions!
