I would ask the
sponsor to provide its version of the form. (Each sponsor is going to have its
own version.)
Such a form can
contain a wide variety of assurances regarding the status of your organization
(and perhaps its “key” personnel) with regard to criminal
proceedings and investigations/audits/inquiries by governmental entities. The
phrasing of such assurances can be overly broad, and I would suggest that your
Legal review the substance carefully.
