Here is a transcript of a AAMC Effort Reporting Conference call with Joe Ellis of NIH held on Feb 10, 2005.  I believe the last Q&A deals with your question.  This has been a great resource document for me.

Subject: [RESADM-L] Effort on No-cost extensions

Hello all,
I'm curious to know what your policies are WRT effort on DHHS no cost extensions.  If your PI intends to reduce effort by more than 25% during a 'regular' no cost extension (ie an R01 subject to expanded authorities...carryforward less than 25%), are you requesting approval from DHHS?
Thanks in advance!
Helen
