Can anyone give me a website address to where the PHS 398 forms
are located that I can download, complete the application and save the file?
Currently I can locate the forms on www.nih.gov, but can only download them
and print them out. Then we have to manually type the information on a
typewriter. It would be much to our advantage if we could download, complete
the forms and save the document for future changes or corrections.

 Hi-

 Just to let you know:

 An error has been corrected in the instructions for the PHS (NIH)
398
iPDF.  The original version, which many may have downloaded, contained the
following:

 "Form Page 2 (BB) The "abstract page" has changed significantly from
previous 398 forms. The abstract description is now in a smaller box,
performance sites are listed on this page rather than the face page, and
only key personnel (name, organization, role on project) are now included
on this page. Form Page 3 (CC)"

 To the best of my knowledge, w/r to the items above, there has been
no
change from the 5/95 version.  TRAMS agreed and made the correction.

 Nevertheless, I had one investigator call me and ask about these
"changes", none of which had been mentioned in our Forms/Modular workshop
several weeks earlier.
