We are a small liberal arts college with an active research faculty.  I
don't think my answers to your questions will be all that helpful -- what
I'd really like is a summary of the responses you get!

Our "research adminsitration" function (me!) reports to the Grants Director
who reports to the VP External Affairs -- we have a Development
orientation.  Our annual reports simply list all grants awarded (regardless
of year submitted) + all grants still pending (regardless of whether
they're essentially the same project to more than one sponsor) + all grants
denied (regardless of year submitted = Total Grant Activity.  Any
percentages are done on this basis.

I maintain more details on "faculty grants" (those grants initiated by
faculty to support primarily their own scholarly activity vs. grants that
support an institutional mission that may be written by faculty members)
... and in my reps I do try to show the track record of specific proposals.
I have never tried to compensate for duplicate submissions of the the same
project except in two cases:  I do a report on the success faculty have in
seeking support for their leaves (and that usually means duplicate
submissions) and I have footnoted my report on our success with the NSF-ILI
program to indicate the success of projects because in some cases it took
two attempts to get the funding.

I'm looking forward to finding out what the "industry standard" is ...
