"Character length restrictions" refers to the number of characters
permissible in each of the boxes on the Face Page of the PHS398. On
the Face Page sample, there are emboldened numbers (actually they're
black blocks with white numbers) on every line of data to be entered.
 These are the character length restrictions.  That is, the number of
characters for Title of Project should not exceed 56, the numbers of
characters for the name of the Official Signing (in box 14, first
line) should not exceed 30, etc.

We are confused about the meaning of the item 9b, page 36 in the new
PHS 398 application packet.  How do you interpret the last sentence,
"Follow the character length restrictions noted on the sample face
page."  The problem is we don't find a reference to conflict of
interest on the face page or any connection to the statement.
What format are other institutions using to notify DHHS of financial
conflicts of interest?
