The only other comments I offer, prior to determining if a 3rd request is legitimate, are:
1)  Examine the level of integrity of your PI - Are NCEs a general pattern with the PI? Is the PI over committed? Has the PI been devoting sufficient effort to the project? Has reasonable progress been made? etc...
2)  Examine the circumstances for why the work has not yet been completed - reasonableness, situations beyond the PI's control, etc.

I’ve had a couple of PIs request a 3rd NCE from NSF. I’ve always counseled them that they need to have a frank conversation with their program manager about the situation. Both requests were successful because of that conversation. The program manager normally wants to make sure the work is able to be completed.

What’s everyone’s experience with 3rd no-cost extensions from NSF?  Have you seen them granted?  I’ve never had a 2nd year turned down, but none of my PIs have ever requested a 3rd year.  I told the PI in question that he might as well go for it—the worst they can say is no—but it would be good to have some idea of what his chances are as we plan for close-out in the event it’s denied.  It’s an IOS program award, in case that makes any difference in your experience.
