My question concerns how your institution's (mostly public universities) IRB deals with exempt projects. What documents (if any) does your IRB require for exempt projects: an application form? (e.g., to verify that the project is exempt), an informed consent statement or signed informed consent forms? Are the requirements any different for exempt projects involving minors? Specifically, what is required in terms of consent documents for projects involving minors that ARE part of regular classroom activities (category 1), such as program evaluation?  Does your institution require parental consent and child assent?  We are in the process or revisiting our own guidelines and are researching how other IRBs are handling exempt projects.
