It has happened to me. Luckily, we knew where he was (Old Dominion), and we tracked him down and made him write the report. All payments and new awards to our institution from
the agency were cut off until he got his act together. PS - agency in this case was NOAA.

On a related note - I've been going into the Fastlane Institutional Reports Module and seeing who it shows as having delinquent reports, and making sure the reports are in.
Many of them were submitted, but NSF doesn't seem to have them on file, so we're re-submitting the reports.
